Students have the responsibility to notify the Office of the Registrar each time their personal contact information changes. Updates can be made in person, via Workday, or by submitting the completed form by mail or fax.

ISU employees (graduate assistant or student worker), must report an address change correction directly to University Human Resources, 3810 Beardshear Hall. 

Request for verification of student information (e.g. enrollment, degrees, academic standing, fees paid, GPA, etc.). Student's signature required on the form.

Students use forms from the Department of Education to defer repayment of their loan(s). Students must certify that they are at least a half-time student.

The DMACC/Iowa State University Cross Enrollment Program provides full-time undergraduate students at both institutions the opportunity to enroll in one class, for credit, without paying tuition at the partner institution.

ISU students registering at DMACC must complete the DMACC Cross Enrollment Application and Registration form and checklist.

Students who want to change curriculums/majors use this form to complete the process.

Students must provide this form with an original signature (electronic signatures will not be accepted) to receive their grades in the mail. The form may be submitted in person or by fax. A new form must be completed each academic year.

Graduate College, Veterinary Medicine, and Undergraduate students must complete the appropriate graduation application form. Submit the completed form (remember to sign the form) to the office indicated on the application by the first week of class for the term you intend to graduate.

Students registering for classes under the Postsecondary Enrollment Options Act (PSEO) will use this form.

Students who plan to return to Iowa State University after an absence of 12 months or more must submit this form

  • Undergraduates, international undergraduates, and international graduate students must use the appropriate form and procedures.
  • Before completing this form, review the information about reentry.
  • Complete and sign the printed form.

Students must notify the Office of the Registrar of their intent to cancel their registration, using the Registration Cancellation Form. 

  • Additional information about registration cancellation is listed on the form and in the catalog
  • Cancellations will be posted at least 1-2 working days after processing. 
  • Complete and sign the printed form, then submit it to the Office of the Registrar.

A Trial Schedule Time Grid is provided for students to plan their schedules. The grid can be downloaded and completed in writing or online.

Students can use the worksheet to plan their schedules before registering for classes. This form can be completed online and printed.

Students who would like to be classified as an Iowa resident for tuition and fees must complete this form after they have fulfilled the residency requirements. This form can be completed online. Submitted forms must include original signatures.

Students who have not been charged the Activity and Service Fee but wish to access Recreation and Services facilities and programs can request the fee assessment through Workday.

Official transcripts are available to current and former students at no cost though our Parchment third-party service partner. Create a Parchment account to order your transcript. Current students can access Parchment on their Academics Dashboard in Workday.

Withdrawing from the university means you are dropping ALL your classes on or after the first day of the term. To withdraw, you must contact your advisor or major professor to initiate the withdrawal process.

Per the student’s request, the “Request for Withdrawal” form is initiated and submitted by the student's college to the Office of the Registrar. The student is withdrawn from all courses based on the withdrawal date on the form, and tuition and fees are adjusted if appropriate according to established policies approved by the Board of Regents, State of Iowa.
